Consent Form for Students

I understand that:

  • I will be doing a survey about what children and young people think and believe about religion and spirituality;
  • Participation in this survey is voluntary which means I do not have to take part if I don’t want to and nothing will happen to me;
  • There are no right or wrong answers and if I don’t want to answer some of the questions that is fine;
  • I can stop at any time and this will not upset anybody;
  • My answers might be included in a report written by the research team, but no-one will know that it was me saying these things because my name and the name of my school wont be used;
  • If I have any worries about the discussion then I can talk about these with my teacher, Brad, Richard or Anne.

 

The ethical aspects of this study have been approved by the Southern Cross University Human Research Ethics Committee. The Approval Number is ECN-05-126.
